On August 27, 2011 we moved our office location.� We were at the previous location since 1980.� The lease was up and we took the opportunity to move to a larger, nicer, updated, clean, brand new space.� We are very excited about how it turned out and hope you will be too.

 

With great sorrow we inform you of the passing of our friend and colleague � Dr. Stuart Eichenfield.� He passed away on May 30, 2008.� He will be missed but not forgotten.� He was a true inspiration and a wonderful physician, friend and mentor.� His wife and our office manager for many years, Fran Eichenfield passed away June 13, 2011.� She too will be missed.

In doing so, each patient (or patients' parent or legal guardian) will be given a "HIPAA PRIVACY NOTICE" to keep.� This notice provides you with information about your rights and our legal duties and privacy practices with respect to the privacy of protected health information. This information may be updated and changed from time to time.� The most current privacy notice will be posted in the office waiting room and on the web site.� Our privacy office is Dr. Shear.

 

As a result of the HIPAA regulations, we can not release any of your medical information without your written consent.� This includes all school and camp forms.� The release form is available at our office or can be downloaded if you click on "Authorization for completing school/camp form".� Upon request this form can be faxed to you.� We apologize for any inconvenience.
